Procedural Requirements
Out-patient Services in Areas with Fortune Care Owned Clinics:
  • Go to any Fortune Care Clinic in Metro Manila or in areas with provincial branches
  • Present your Fortune Care membership card for the necessary consultation/treatment of your illness
  • For conditions requiring elective or non-emergency confinement, secure approval from the clinic director
Out-patient Services in Areas without Fortune Care Owned Clinics:
  • Proceed to the hospital coordinator of the outpatient department/industrial department
  • Present Fortune Care membership card for the necessary consultation/treatment of your illness
Out-patient Emergency Care in Accredited Hospitals:
  • Proceed to the emergency room of the nearest accredited hospital.
  • Present Fortune Care Membership Card
  • Fill-up hospital data form which will be forwarded by the hospital to Fortune Care
Out-patient Emergency Care in Non-Accredited Hospitals:
  • Proceed to the Emergency room of the nearest non-accredited hospital
  • Pay hospital bills and professional fee and secure all the documents
  • File reimbursement at Fortune Care branch/head Office within 10 days from date of discharge
  • Fortune Care will reimburse 80% of the total approved hospital bills
Non-Emergency Hospitalization in Accredited Hospitals:
  • Obtain UMRC approval in Metro Manila and areas with Fortune Care branches for admission involving major elective surgery or special diagnostic procedures
  • Present admitting order of Fortune Care doctor to admission section of accredited hospital together with your Fortune Care membership Card.
  • You will be admitted according to your room and board plan
  • You will be given special assistance by Fortune Care through our Liaison Officer
  • Before discharge, proceed to the Billing and Collection Department to sign your statement of account; pay excess charges, if there are any.
  • Secure a duly accomplished Philhealth form from your company and submit to the same section
Emergency Hospitalization in Accredited Hospitals:
  • Proceed to the emergency room of the nearest accredited hospital
  • Present Fortune Care membership card
  • Call nearest Fortune Care Medical Office within 24 hours to report confinement at 1-800-10-633-8888 (PLDT), 637-4229 or 6370872 to 77
  • You will be given special assistance by Fortune Care through our Liaison Officer
  • Before discharge, proceed to the Billing and Collection Department to sign your statement of account; pay excess charges, if there are any
  • Secure duly accomplished Philhealth form from your company to be submitted to the same section
Emergency Hospitalization in Non-Accredited Hospitals:
  • Proceed to the emergency room of the nearest non-accredited hospital
  • Pay hospital bills and professional fees and secure all documents
  • File reimbursement at Fortune Care branch/head office within 10 days from the date of discharge
  • Fortune Care will reimburse 80% of the total approved hospital bills
PhilHealth Members
A confined member, who is a PhilHealth member or a dependent of such a member, must file his claim for benefits prior to discharge; otherwise, he shall pay for the PhilHealth portion of his total hospital bill.
