MEMBERSHIP ELIGIBILITY - Any nominee that the payor will enroll in the program will be accepted provided within the age range of at least 90 days old but not more than 65 years old.

PAYOR - The payor is based in the United States, or any other country other than the Philippines.

INDIVIDUAL PROGRAM Beneficiary - Any nominee that the payor will enroll in the program will be accepted provided within the age range of at least 90 days old but not more than 65 years old.

FAMILY PROGRAM Beneficiaries - Hierarchy shall be strictly followed for FAMILY enrollees.

    For married payors: He / she should enroll first his / her spouse and then the eldest child, then the second eldest and so on.
    For single payors: He / she should enroll first his / her mother, then the father, and then the eldest sibling and so on; Single Parents can only enroll his / her children.

NOTE:

  1. Hierarchy shall be strictly followed for FAMILY enrollees. For married payors, he/she should enroll first his/her spouse and then the eldest child, then the second eldest and so on.   2. Enrollees who are not satisfying the required hierarchy for family enrollment shall automatically be treated and rated as INDIVIDUALS.
  3. All rates are inclusive of E-VAT as mandated by the Philippine government following the AHMOPI formula. If at any future time, additional taxes, levies and government obligations are imposed to Philippine HMO's, such additional charges shall be done by the payors.
